Project: South Bossier Medical
Bids will be received at North Caddo Medical Center, 815 South Pine Street, Vivian, Louisiana until 9:00 a.m. on Tuesday, August 22nd, 2023, in the Administration Conference Room for all labor, materials, and equipment required to fully complete the project listed below. The project will be awarded to the lowest qualified, responsible, and responsive contractor. The bid must be submitted in a sealed envelope.
All bids received on time will be publicly opened and read aloud. Site visits can be arranged through the engineer’s office.

Project Summary: This project will be to install access, general site work, parking, and utility work for future install of a modular medical clinic.

Printed copies are not available, but arrangements can be made to obtain them through most reprographic firms. Plan holders are responsible for their own costs. Documents shall be available to Bidders on June 15th. Addendums will be distributed via electronic form at ncmcla.com/public-bid-notice/
Bids will be accompanied by a certified check, cashier’s check or bid bond payable to North Caddo Medical Center, the amount of which shall be five percent (5%) of the total of the project. Money Orders will not be accepted. This shall be given as guarantee that the bidder will execute the contract, if it were awarded to him, in conformity with the Contract Documents. Bid Bonds shall meet the requirements according to R.S. 38:2218.
Bidders check list items include Completed Louisiana Uniform Bid Forms, Completed Bid Bond or Bid Security – AIA Document A-310 (Circular 570 as amended) 5% of total bids, Acknowledgement of Addenda, and name of firm as required by R.S.38:2212(B)(2). The following can be submitted at a later date by the apparent low bidder: Completed Section 000320 – Subcontractor, Supplier, and Manufacturer List, Completed Section 000450 Non-Collusion Affidavit, and Completed Section 000520 – Safety Regulations Agreement Form.
The successful bidder shall be required to furnish a Performance and Payment Bond in an amount equal to 100% of the Contract written by a company licensed to do business in Louisiana, and who is currently on the U.S. Department of the Treasury Financial Management Service List or by Louisiana domicile insurance company with at least an A-Rating in the latest printing of the A.M. Best’s Key Rating Guide to write individual bonds up to ten percent of policyholder’s surplus as shown in the A. M. Best’s Key Rating Guide and complies with R.S. 38:2219 (R.S.28:2218 Bid Bond). The bond shall be countersigned by a person who is under contract with the surety company or bond issuer as an agent of the company and who is licensed as an insurance agent in this State and who is residing in this State.
Bids shall be accepted only from contractors who are licensed by the State of Louisiana for the classification of Building Construction. No bid may be withdrawn for a period of sixty (60) days after receipt of bids, except under the provisions of La.R.S. 38:2214.
The successful bidder shall supply all required insurance with carrier(s) acceptable to North Caddo Medical Center.
The Owner reserves the right to reject any and all bids for just cause. In accordance with La.R.S. 38:2212 (A)(1)(b), the provisions and requirements of this section, those stated in the Advertisement for Bids, and those required on the bid form shall not be considered as informalities and shall not be waived by any public entity.
