Notice to Bidders
Hospital Service District No. 1 Caddo Parish
D/b/a North Caddo Medical Center
All bids will be received at NCMC Administrative Conference Room on May 16th, 2023 at 1pm for materials, and
equipment required to fully complete projects listed below. Project will be awarded to the lowest qualified,
responsible and responsive contractor(s). When submitting the bid, each bid must be submitted in a sealed
envelope containing all requested items in the bid check list.
All bids received on time will be publicly opened and read aloud on May 16th, 2023 at NCMC Administrative
Conference Room (815 South Pine Street, Vivian, Louisiana).
All bidders are will need to attend the pre‐bid conferences as indicated below. Attendance at these conferences is
mandatory. By submitting bids, bidders acknowledge responsibilities and familiarities with the project sites.
1. North Caddo Medical Center Annex Building
Location: 1003 Spruce Street. Vivian LA 71082
Project Summary: the work includes the abatement of and renovation of a single story 11,822 s.f. existing
office building into a new pharmacy and dental clinic. The project also includes the addition of 7671 s.f. of
new construction. The existing slab is to remain. All MEP systems will be replaced. Significant site
improvement are expected.
Mandatory Pre Bid Meeting: April 25th 2023 at NCMC Administrative Conference Room (815 South Pine
Street, Vivian, Louisiana).
Estimated construction cost and duration: $4 million / 10 months

Complete Bid Documents are available in electronic form. They may be obtained from the following link: Printed
copies are not available from the Architects, but arrangements can be made to obtain them through most
reprographic firms. Plan holders are responsible for their own costs. Documents shall be available to Bidders on
April 7th 2023. Addendum will be uploaded to the same link listed above. Contractors are responsible to check
availability prior to bidding.
Bids will be accompanied by a certified check, cashier’s check or bid bond payable to North Caddo Medical Center,
the amount of which shall be five percent (5%). Money Orders will not be accepted. This shall be given as
guarantee that the bidder will execute the contract, if it were awarded to him, in conformity with the Contract
Documents. If Bid Bond is used, it shall meet the requirements according to R.S. 38:2218.
Bidders check list items include;
- Louisiana Uniform Bid Form, Completed Bid Bond or Bid Security ‐ AIA Document A‐310 (Circular 570 as
amended) 5% of total bid, Acknowledgement of Addenda, and name of firm as required by
R.S.38:2212(B)(2).The following can be submitted at a later date by the apparent low bidder: - Completed Section 000320 ‐ Subcontractor, Supplier, and Manufacturer List,
- Completed Section 000450 Non‐Collusion Affidavit, and Completed Section 000520 ‐ Safety Regulations
Agreement Form. - (3) example projects with references for each project.
- Copy of Louisiana Contractors License
The successful bidder shall be required to furnish a Performance and Payment Bond in an amount equal to 100% of
the Contract written by a company licensed to do business in Louisiana, and who is currently on the U.S.
Department of the Treasury Financial Management Service List or by Louisiana domicile insurance company with
at least an A‐Rating in the latest printing of the A.M. Best’s Key Rating Guide to write individual bonds up to ten
percent of policyholder’s surplus as shown in the A. M. Best’s Key Rating Guide and complies with R.S. 38:2219
(R.S.28:2218 Bid Bond). The bond shall be countersigned by a person who is under contract with the surety
company or bond issuer as an agent of the company and who is licensed as an insurance agent in this State and
who is residing in this State.
Bidders Experience: Bidder shall have and provide references for three (3) projects of new construction and/or
renovation of medical, pharmacy or dental clinic. Bidders without previous experience and references shall be
disqualified.
Bidders may inspect the existing site conditions Monday through Friday 8:00 am until 2:00 pm; call hospital for
appointment.
Bids shall be accepted only from contractors who are licensed by the State of Louisiana for the classification of
Building Construction. No bid may be withdrawn for a period of sixty (60) days after receipt of bids, except under
the provisions of La.R.S. 38:2214.
The successful bidder shall supply all required insurance with carrier(s) acceptable to North Caddo Medical Center.
The Owner reserves the right to reject any and all bids for just cause. In accordance with La.R.S. 38:2212 (A)(1)(b),
the provisions and requirements of this section, those stated in the Advertisement for Bids, and those required on
the bid form shall not be considered as informalities and shall not be waived by any public entity.
